Pharmacological cardioversion in atrial flutter

The drugs which can revert atrial flutter to sinus rhythm include:

  • sotalol
  • flecainide
  • disopyramide
  • propafenone

(Ibutilide and dofetilide, which are able to achieve very rapid cardioversion, are not currently available in the UK.)

It should be remembered that any antiarrhythmic can induce arrhythmia - in this case the ventricular response to the atrial activity may occasionally be accelerated.
